Application Submission

When submitting an application in eSirius, please be sure to answer all of the questions completely. Any questions should be directed to the IACUC office iacuc@ocm.utah.edu.

The IACUC office will perform a prescreen to review completeness and regulatory content.  The IACUC office will assign a veterinarian to perform the veterinary review at the time of the protocol prescreen.  The assigned veterinarian will have 5 business days to perform the veterinary review and submit the review to the IACUC office.  The IACUC office will merge the protocol prescreen questions and the veterinary review questions and send the application back to the PI.  The PI will need to respond to the questions in the eSirius electronic protocol form and revise any application sections.  Upon completion of answering the prescreen and veterinary questions and the making the necessary protocol revisions, the PI will then again submit the protocol back to the IACUC office.

The application will either be reviewed at the next convened meeting by the Full Committee (FCR) or will be reviewed via Designated Member Review (DMR).  When the Committee, FCR or DMR, requests a response (clarification answer) from the PI, the PI must respond and revise the application as applicable and submit the document back to the IACUC office.  Final disposition of the application will take place once the response is received and/or the application will be placed on the agenda for review at the next convened meeting.

When the Committee requests a response from the PI who has submitted a 3-year renewal protocol, it is the PI’s responsibility to respond in sufficient time for the Committee to review and approve the response before the current protocol expires.  Please allow plenty of time for the review process.  Animals cannot be ordered from the previous protocol once it has expired.  Animals on study at the time of protocol expiration will be transferred to the veterinary holding protocol in which no research can be conducted.  Protocol extensions beyond the original 3 year approval date are not permitted by regulatory agencies.

All 3-year renewal protocols must be submitted through the electronic protocol application, eSirius3G. Please contact the IACUC staff iacuc@ocm.utah.edu to receive training on using eSirius3G.  3-year renewal notices are generally sent out 90 days prior to the protocol’s expiration date.

The replacement protocol must submitted by the deadline stated.  After final approval of the new protocol is given, the protocol number will remain the same.  Not responding to the expiration notice can result in the inactivation of the protocol. If the protocol expires, you will no longer be authorized to perform work or use animals.  Animals on study at the time of protocol expiration will be transferred to the veterinary holding protocol in which no research can be conducted.
